Description/Purpose:

Under the Rajya Vyadhi Nidhi scheme, an Illness Assistance Fund society has been set up to provide treatment for Below Poverty Line (BPL) population of the State.

 
Particulars:

Under this scheme, BPL patients can avail of treatment free of cost for the following fatal diseases: Cancer, Heart Disease (with surgery), Severe Mental Disorder (with surgery), Brain tumour (with surgery), H.I.V/AIDS, Total hip & knee replacement, Spinal surgery, Major vascular surgery, Bone marrow transplantation, Cornea Pasty and Kidney transplantation.

The patients can avail of treatment at the following 12 empanelled medical institutions:
1.      All India Institute of Medical Scence (AIIMS), New Delhi
2.      Medical College, Meerut
3.      Sanjay Gandhi Post Graduate Institute of Medical Sciences (SGPGI), Lucknow
4.      Post Graduate Institute of Medical Education and Research (PGI), Chandigarh
5.      Sarojini Naidu Hospital, Agra
6.      Irvin Hospital, New Delhi
7.      Safdarjung Hospital, New Delhi
8.      Mental Hospital, New Delhi
9.      Mental Hospital, Agra
10. Mental Hospital, Bareilly
11. Himalayan Institute Hospital Trust, Dehradun
12. Dr. Sushila Tiwari Memorial, Forest Hospital and Medical College, Haldwani
Under this scheme, health cards are issued to BPL families for free treatment in government hospitals. Special provision has also been made for providing a subsidy up to 25% (in remote areas) and 35% (in the remotest areas) on the total cost of setting up a medical clinic in the remote and very remote hill areas.
Applicants can collect the required form from the Block Health Office or the District Health Office. They can also contact the Chief Medical Officer (CMO) of the District Hospital. The maximum illness assistance fund is Rs. 1.5 lakh.

BPL individuals and families.
 
Eligibility criteria:

The patients must belong to the category of BPL as established by the BPL list at the Block Development Office or the District Administrative Office.
